No-show appointments: what they cost and how to cut them

A client who does not turn up costs more than one unpaid treatment. A yearly loss calculator, what the research says about reminders, and nine fixes that work.

Why a client does not turn up

A client who does not turn up almost never does it on purpose. Most of the time they simply forgot, or the appointment was booked two weeks ago and never made it into their own calendar, or something came up and there was no convenient way to say so. Sometimes they booked in two places and chose. And sometimes, especially when the appointment costs nothing and not coming costs nothing either, it just did not feel important enough.

Each of those reasons has a different fix, which is why there is no single solution. But there is a lot in common: a client who gets a reminder on time, who can cancel in one tap, and who knows in advance what happens if they do not come, turns up far more often.

What it costs: the maths

The direct loss is the price of the treatment that was not given. But that is not all: the time was set aside, the slot was taken in the calendar, and another client who wanted exactly that hour did not get it. Counting only the unpaid treatments already gives a number that surprises most owners.

That number is revenue not received, not profit. It also leaves out your own time: the calls, the gaps nobody can fill at the last minute, and the clients who were turned away.

What the research says

Most research on missed appointments comes from clinics, but the mechanism is the same in every appointment-based business. A systematic review published in Health Policy (Dantas et al., 2018) gathered studies from around the world and found an average no-show rate of 23%, with wide gaps between countries and kinds of service. Your own number is different, but the order of magnitude explains why this is worth working on.

On the other side, a systematic review by Cochrane examined mobile phone message reminders for healthcare appointments and found they improve attendance compared with no reminder. That is the finding that matters: a simple reminder, sent on time, works. It does not promise attendance, but it moves the number.

Nine fixes that work in a small business

  1. A reminder the day before, with a request to confirm. Not "remind" but ask: are you coming? Whoever answers "no" has freed a slot you can fill.
  2. A second, short reminder for long treatments. Two or three hours before, one sentence. Not for every appointment — only for the ones that cost you most.
  3. An easy way to cancel. A link in the reminder, one tap. A client who has to phone to cancel simply does not phone.
  4. A cancellation policy stated up front. On the booking site, in the confirmation message, at the counter. A client who knows the rules respects them more.
  5. A waitlist. A slot that is cancelled anyway is offered automatically to whoever is waiting. We wrote a separate guide on that.
  6. Full payment in advance for expensive treatments. People who have paid turn up. In Torly, a business in Israel sets this on its booking site, and the payment is taken by card when the appointment is booked.
  7. Book the next appointment at the end of this one. A client who leaves with an appointment in hand is a client you do not have to chase, and that appointment gets a reminder like every other.
  8. A personal call to anyone who has missed twice. Not an automatic message. A one-minute conversation finds out what happened and settles what comes next.
  9. A note on the client card. So that the next time they call, you know to ask for payment in advance or to offer an hour that is easy to fill.

Confirmation: one question that saves phone calls

The difference between a reminder and a confirmation request is a question. A reminder says "you have an appointment tomorrow at 10:00". A confirmation request says "you have an appointment tomorrow at 10:00 — are you coming? Yes / No". The first is read and forgotten; the second demands an answer, and that answer is the most useful piece of information you have the day before.

Whoever answered "yes" will probably come. Whoever answered "no" freed a slot in time. And whoever did not answer at all is the only client who needs a phone call, instead of calling everyone.

What to write to a client after a no-show

The message after a no-show is not a punishment. It keeps the relationship, restates the rules, and offers a way forward. Short, no blame, one action.

A sample message after a no-show
Hi {name}, we missed you today at {time}. Hope all is well. You can book a new appointment here: {link}. A small reminder: cancelling up to 24 hours before an appointment costs nothing; after that our cancellation policy applies.

After the message, two small actions: a note on the client card (the date, what happened), and a decision whether this was a first time or a pattern. A first time is forgiven. A pattern needs a conversation.

What not to do

  • Do not shame anyone in public. A social-media post about "clients who don't show up" pushes away every client, not only the one who missed.
  • Do not double-book. Booking two clients for the same hour on the assumption that one will not come works until the day both do. Then you have lost two.
  • Do not punish everyone for a few. Full payment in advance for every appointment, a twenty-minute haircut included, drives away more clients than it saves. Keep it for the expensive treatments.
  • Do not rely on memory. "I remember who doesn't show up" works until a hundred clients. After that you need a note on the card.

What is the difference between a no-show and a late cancellation?

A late cancellation is a client who told you, but too close to the appointment for the slot to be filled. A no-show is a client who did not tell you at all. In both cases the time is lost, but a late cancellation at least gives you a chance to offer the slot to someone else.

When is the best time to send an appointment reminder?

One reminder the day before gives the client time to cancel properly, and that is the cancellation you can fill. For long or expensive treatments, add a short reminder a few hours before. More than two become noise.

Should a reminder ask the client to confirm?

Should I take payment in advance for appointments?

It depends on the treatment. For a long, expensive treatment, full payment in advance at booking is common and cuts no-shows sharply.
